I almost forgot about this change so I have spent a few hours today getting
the change compiling and running against the latest version of source,
adding support to curl.exe for a new --mail_auth argument as well as
updating the documentation as I said I would for v7.23.0 - sorry it's a tad
late ;-)

I also spotted that I hadn't adhered to the RFC correctly in my original
modification so the second patch fixes that as well. It was easier to do
this as a separate patch than produce one consolidated patch ;-)

I haven't been able to get the test cases up and running yet so I was
wondering if you are able to add the --mail-auth option to an existing test
case as we were talking about in October please?
