Summary: Fails to compile on QNX 6

Initial Comment:
cURL fails to build on QNX 6 because

 * strequal.c fails to compile because strcasecmp is not declared. strcasecmp is declared in strings.h, but strequal.c doesn't include it.
 * setmode is called but is not defined because main.c assumes that it's present if O_BINARY is defined.

The attached patch fixes these issues.

The patch looks good to me, but all the relevant platforms that don't use
autoconf will need to be updated to add HAVE_SETMODE. This especially
includes the DOS/Windows platforms, where curl will completely break
without this update.
