I pushed a series of commits this afternoon that fixes the problem for me, and
also that improves some CONNECT test cases to make (bad) connection drops
visible and trigger test failures so that we'll notice the next time I
introduce this kind of regression...

I'd appreciate if someone took the current git code and ran it against a real
proxy server and tried CONNECT with NTLM over it.
